The reservoir was built by damming the Ngrancah River as part of the Integrated Irrigation Sector Project (IISP), aimed to support the Kalibawang irrigation system covering approximately 7,152 hectares of agricultural land. The irrigation network itself connects several irrigation areas across the region.

After around two and a half years of construction, Waduk Sermo was officially inaugurated by President Soeharto on November 20, 1996.
